2026 Mandatory GFL Registration Documents
The following are required for GFL (tackle) registration only: birth certificate, 2026 physical form, and baseline concussion testing for 11 & 12 year olds / 8th graders.
Physical form information (per GFL Sports):
- The examination/physical must be performed in 2026.
- The signed 2026 GFL physical form must be submitted to your Team Mom before any GFL activity.
- No photocopies. Originals only, and every line on the form must be completed. No exceptions.
2026 Mandatory GFL Equipment
Required for tackle football only. 7v7, flag football and the lineman challenge typically require only a mouth guard and cleats, with soft-shell helmets recommended but not required. Questions? Email football@lanieraa.com.
- Football helmet (regulation white w/ white facemask)
- Shoulder pads (regulation youth)
- Knee pads
- Thigh pads
- Hip pads
- Tail pads
- Football cleats
- Mouth guard
- Chin strap

2026 Concussion Baseline Testing
The athlete must complete the testing themselves and save the result as a PDF at the end.
This is not a pass or fail test, and parents should not take it for their child or help with answers. The point is to establish a baseline for how the child's brain functions now, so a physician can repeat the test after any concussion and compare results.
